That’s a question we ask ourselves every day at Crossroads Hospice. “Why not?” helps us find new and better ways to deliver hospice care to our patients, their families, caregivers, and health care providers. We know hospice is very important and meaningful care, that it is largely misunderstood and underutilized. We set out to help more people access hospice and help more people experience a better way. That’s why we developed our one-of-kind programs like:

Gift of the Day: to help our patients experience their perfect day, whatever that looks like for you. Our staff, volunteers, and local businesses work together to bring these “Gifts” to life.

Gift of Blessings and Gratitude: to help our patients be remembered, to tell their story, to matter. By creating a Life Journal, they can pass on their love, experience, and advice to their family.

Our gift to our community, Grief Recovery Method Outreach: Led by bereavement counselors that help participants effectively cope with the pain caused by loss, this service is open to all.

Our commitment to patient care is probably best demonstrated by our Evenmore Care program. It’s the commitment from our staff made possible with lower than average patient-to-staff ratios which ensure we see our patients virtually every day. With this level of increased patient contact, our staff can identify changes in a patient’s condition and initiate the Watch Program, a special care level we created to identify increased or sudden decline. Our staff is able to monitor and respond to patient developments through multiple daily visits, preventing re-hospitalizations and to be there when patients and families need us most.

At Crossroads we recognize the sacrifices the armed forces have made in serving their country. Because we believe the American people owe a debt of gratitude for the security of this nation to those who served to defend it, we developed our Veterans program to better care for our veterans.

Why Now?

Because you’re one patient, one family, and we are one team: physicians, nurses, medical social workers, certified pastoral education chaplains, bereavement staff, certified home health aides, volunteers, pharmacist, medical equipment specialist, insurance claim processors, and office support staff. Your Crossroads team is here to encompass the physical, psychological, and spiritual needs of terminally ill patients, while supporting the wellbeing of caregivers. We come to you, providing care wherever you reside: your home, long-term care facility, or hospital. Care is provided on the basis of need, not the ability to pay. We know that patients who chose hospice care live longer than similar patients who do not.
