Her dissertation, Transitional Care in Rural Community: The Effects on Reductions of Readmissions, states the dangers of patients who are released from the hospital without the benefits of transitional care. “Patients frequently do not understand their medication dosing or indications, and they are sent home with missing prescriptions or duplicate therapy due to complex medication dosing; they do not understand how to take care of themselves and monitor for exacerbation early to avoid readmission.”
