The Twin Creeks Hospital – “Admit from Home” evaluation process:

  • Starts with your call to the admissions coordinator requesting an “in-home” evaluation
  • An “in-home” evaluation is scheduled by a clinical staff member from Twin Creeks Hospital
  • We collection pertinent medical information from you and your physicians
  • If appropriate, an order to admit for intensive rehabilitation is written by your physician, in consultation with the medical director from Twin Creeks Hospital
  • Approval from the health insurance company, when required

We understand that the mere transportation process to get someone suffering from any of the previously listed symptoms to a physician’s office is a difficult task in and of itself and is strenuous on the individual.
