What is Post Hospital Rehabilitation?

Post hospital rehabilitation is also referred to as “post-acute,” “sub-acute” and “transitional rehabilitation.” After an acute condition and facing discharge from the hospital, transitional rehabilitation provides the ideal setting for care and rehabilitation services like physical and occupational therapy. At Santé of North Scottsdale, our primary focus is a successful rehabilitation outcome and transition home!

We accept Medicare and most health plans. Often, Medicare pays for your entire stay after a qualifying hospital stay.
